$93,600 – $115,500 annually

Position Overview

As a  Registered Nurse (RN) , you will provide skilled nursing care to patients in their homes, working under physician orders and care plans, while collaborating with a dedicated clinical team.

Service Areas:

• Denver
• Broomfield & surrounding communities
• Northern Colorado
• Southwest Colorado

Key Responsibilities

• Deliver skilled nursing care based on individualized care plans
• Conduct initial patient assessments and ongoing evaluations
• Coordinate care and communicate changes with physicians and team members
• Educate patients and families on care and health management
• Maintain accurate, timely clinical documentation (including OASIS)
• Participate in care planning, case conferences, and quality improvement efforts
• Supervise LPNs, CNAs, and HHAs per regulatory guidelines
• Support patients’ rehabilitative and preventive care needs

Qualifications

• Active RN license
• Graduate of an accredited nursing program
• At least 1 year of nursing experience (preferred)
• Strong clinical judgment, communication, and organizational skills
• Current CPR certification
• Ability to work independently and manage time effectively
